Terms of Service

1. Agreement to these terms

These Terms of Service (the “Terms”) govern your use of the Eazelo website, verification tools, portal, reports, and related services (together, the “Service”). By using the Service — with or without an account — you agree to these Terms. If you use the Service on behalf of a company, you represent that you are authorized to bind that company, and “you” includes it.

2. What the Service is (and is not)

Eazelo compares information you submit about United States healthcare providers against official public records — such as the NPPES/NPI registry, CMS datasets, and state licensing sources — and produces reports that describe what those sources showed at the time of the check.

Eazelo is not a state licensing board, a credentialing organization, or a consumer reporting agency, and an Eazelo report is not an official license certificate, credentialing determination, or legal advice. Public sources can be incomplete, delayed, or inconsistent; where records disagree, reports show the discrepancy rather than resolving it. You are responsible for confirming any final credentialing, employment, or eligibility decision with the issuing board or the authority your use case requires.

3. Eligibility

You must be at least 18 years old to use the Service. The Service is intended for professional and business use in connection with United States healthcare providers.

4. Accounts

You are responsible for the accuracy of your account information and the security of your credentials. Notify us promptly at support@eazelo.ai if you suspect unauthorized use. Accounts are for a single individual; company workflows are provisioned separately.

5. Allowances and fair use

Free anonymous checks, free-account checks, and paid tiers each carry usage allowances, which are shown in the product and may change over time. You agree not to circumvent allowances, rate limits, or access controls — including by rotating identities, automating requests, or scraping the Service.

6. Acceptable use

You agree not to:

  • submit patient information or any protected health information (PHI); the Service verifies provider credentials, not patient data;
  • use the Service for unlawful discrimination, harassment, stalking, or any unlawful purpose;
  • misrepresent an Eazelo report as an official certification, alter a report, or present a report for a provider other than the one checked;
  • resell, sublicense, or bulk-redistribute reports or underlying data without our written agreement;
  • probe, disrupt, or attempt to gain unauthorized access to the Service or other users’ data.

7. Reports and verification codes

Reports include source citations, timestamps, and a verification code so recipients can confirm a PDF matches the record Eazelo saved. Reports and their verification records expire on the schedule shown in the product. A report speaks only as of its generation time.

8. Subscriptions and billing

Paid tiers are billed through our payment processor (Stripe) on a recurring basis until cancelled. You can cancel at any time, effective at the end of the current billing period; fees already paid are non-refundable except where required by law. We may change prices or allowances prospectively with notice before your next renewal.

9. Your data and our license to process it

You retain your rights in the information you submit. You grant us a license to process it as needed to operate the Service — running checks, generating and storing reports, maintaining your history, and preventing abuse — consistent with our Privacy Policy.

10. Third-party sources and services

The Service relies on third-party and government sources (including NPPES, CMS, state licensing sites, and mapping providers) that we do not control. Their availability and accuracy may change without notice, and their own terms may apply to your use of their content.

11. Disclaimer of warranties

The Service is provided “as is” and “as available.” To the fullest extent permitted by law, we disclaim all warranties, express or implied, including merchantability, fitness for a particular purpose, non-infringement, and any warranty that results will be accurate, complete, or uninterrupted.

12. Limitation of liability

To the fullest extent permitted by law, Eazelo will not be liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, revenue, or data. Our total liability for any claim arising out of the Service is limited to the greater of the amounts you paid us in the twelve months before the claim or one hundred US dollars ($100).

13. Termination

You may stop using the Service or delete your account at any time. We may suspend or terminate access that violates these Terms or creates risk for the Service or other users; where practical, we will notify you and, for paid tiers, refund any prepaid fees covering the period after termination by us without cause.

14. Governing law and disputes

These Terms are governed by the laws of the State of Delaware, USA, without regard to conflict-of-law rules. Disputes will be resolved in the state or federal courts located in Delaware, and both parties consent to their jurisdiction.

15. Changes to these Terms

We may update these Terms as the Service evolves. Material changes will be announced on this page with an updated date, and continued use after changes take effect constitutes acceptance.
